Semaglutide: A Borrowed Gut Signal

A long-acting analogue of the gut hormone GLP-1, engineered to circulate for a week instead of two minutes — the most heavily trial-tested messenger on this desk.

The short version

Semaglutide is a synthetic peptide built to copy a hormone your gut already makes — GLP-1, short for glucagon-like peptide-1, released after a meal to tell the pancreas and brain that food has arrived. Semaglutide activates the same receptor (GLP-1R) that natural GLP-1 does, but chemical tweaks let it circulate for about a week instead of about two minutes, which is why it is dosed once weekly.

It is FDA-approved for type 2 diabetes, chronic weight management, reducing cardiovascular events in adults with established heart disease and excess weight, and, since 2025, a liver condition called MASH. In the STEP 1 weight trial, it produced an average body-weight change of -14.9% at 68 weeks versus -2.4% for placebo [4]. In a large cardiovascular-outcomes trial it cut major adverse cardiac events by 20% relative to placebo [3]. What it is

Semaglutide is a 31-amino-acid analogue of human GLP-1, sharing about 94% of its sequence with the natural hormone. Two changes make it a durable messenger rather than a fleeting one: at position 8, a substituted amino acid (Aib) blocks the enzyme that normally chews up GLP-1 within minutes, and at position 26 a fatty side chain is attached that binds reversibly to albumin, the most abundant protein in blood. That albumin binding is the whole trick — it shields the peptide from the kidneys and from further enzymatic breakdown, stretching what should be a two-minute signal into a roughly one-week one. The oral tablet form uses a separate absorption-enhancing ingredient and needs to be taken on an empty stomach, since its own bioavailability is under about 1%.

How it works

As a messenger, semaglutide's job is to occupy the GLP-1 receptor wherever it appears. In the pancreas, that means potentiating insulin release only when blood sugar is already elevated (which limits the risk of dangerously low sugar) and suppressing glucagon, the hormone that raises it. In the stomach, GLP-1 receptor activation slows gastric emptying, which blunts post-meal blood-sugar spikes and is also a major reason for the nausea some people experience.

The weight-loss signal, though, is mostly a brain story. Semaglutide reaches receptors in the hypothalamus and brainstem — regions like the arcuate nucleus and area postrema — where it turns up appetite-suppressing neurons and turns down appetite-driving ones. That is the mechanistic basis for the "food noise" effect people describe: it is not primarily a stomach-fullness signal, it is a change in the brain's appetite circuitry itself [6].

What the research shows

Weight management. In the STEP 1 trial of 1,961 adults with overweight or obesity, weekly semaglutide 2.4 mg produced -14.9% mean body-weight change at 68 weeks versus -2.4% with placebo [4].

Cardiovascular outcomes. In SELECT, a trial of 17,604 adults with established cardiovascular disease and overweight or obesity but no diabetes, semaglutide reduced the composite of cardiovascular death, non-fatal heart attack, or non-fatal stroke by a hazard ratio of 0.80 versus placebo — a 20% relative reduction [3]. An earlier trial in people with type 2 diabetes and high cardiovascular risk found a similar cardiovascular benefit, alongside a signal for worsened diabetic-retinopathy complications [7].

Kidney outcomes. In the FLOW trial of 3,533 adults with type 2 diabetes and chronic kidney disease, semaglutide reduced major kidney-disease events by a hazard ratio of 0.76 — a 24% lower risk [2].

Head-to-head against a newer messenger. In SURMOUNT-5, tirzepatide — which engages two receptors instead of one — produced significantly more weight loss than semaglutide over 72 weeks (-20.2% vs -13.7%, P<0.001), which is the honest context for reading semaglutide's results today [1].

Mechanism. Rodent studies mapped the central appetite circuits semaglutide engages, tracing the weight effect to hypothalamic and brainstem action rather than reduced energy expenditure [6].

Safety review. A dedicated safety review found an overall favorable risk-benefit profile, dominated by transient gastrointestinal effects, alongside an increased risk of gallbladder disease and unresolved pancreatic- and thyroid-cancer signals that low event counts have not yet allowed researchers to confirm or rule out [5].

Reported effects, cautions & safety

Anecdotal, not clinical evidence — the effects below come from patient-community reports and comment threads, not controlled trials, and none of them is a recommended dose.

The effect people describe most often is a quieting of what reviewers call "food noise": the constant background thinking about the next meal goes quiet, often within the first week or two. Reduced cravings for sugar and fried food follow closely behind, and steady weight loss over months is the most consistently reported outcome. Among people using it for diabetes, better blood-sugar readings are a common theme, and a number of reviewers describe an unexpected drop in interest in alcohol.

On the adverse side, nausea is the single most-reported complaint — roughly a third of reviewers mention it, usually worst in the first weeks and after dose increases. A distinctive, frequently mentioned complaint is sulfur-smelling burps, often alongside bloating. Constipation and diarrhea (sometimes alternating), acid reflux, early fatigue, and mild injection-site reactions round out the common list; a smaller group describes hair shedding and a more gaunt facial appearance tied to the pace of weight loss rather than to the drug itself.

Cited safety cautions from the clinical literature: gastrointestinal intolerance is the leading driver of discontinuation, concentrated during dose escalation [5]. A boxed warning for medullary thyroid carcinoma stems from rodent tumor data; human risk remains unconfirmed [5].
